Job SummaryThe Home Health RN provides skilled nursing care across a multi‑county territory, completes OASIS documentation, coordinates care with the interdisciplinary team, and supports quality, compliance, and patient‑outcome goals. This role requires strong clinical judgment, time management, and the ability to work independently in the field.
Schedule- Monday–Friday: 8:00 AM–4:30 PM
- On‑Call: 1 weekend every ~4 weeks
- Weekday Call: 5–6 evenings per month
- Mandatory Meetings:

Wednesdays at 8:30 AM - Daily Morning Call: 8:30 AM via Teams
- Weekly Case Conference:
- Deliver comprehensive home‑based nursing assessments, interventions, and patient education.
- Perform complete medication reconciliation at every visit.
- Develop, implement, and update individualized care plans.
- Monitor patient progress and communicate changes to physicians and the care team.
- Schedule and coordinate visits across Bourbon, Clark, Estill, Fayette, Franklin, Harrison, Jessamine, Nicholas, and Woodford counties.
- Provide high‑need coverage in Estill, Clark, and Nicholas counties.
- Adapt to census changes, referral volume, and staffing needs.
- Complete SOC, ROC, Recertification, Follow‑Up, and Discharge assessments.
- Submit accurate OASIS documentation within 24 hours.
- Ensure documentation supports clinical necessity, compliance, and reimbursement.
- Follow Medicare Conditions of Participation, state regulations, and agency policies.
- Maintain infection prevention, patient safety, and risk‑management standards.
- Participate in quality improvement and audit activities.
- Collaborate with physicians, therapists, social workers, and office staff.
- Communicate patient needs and care plan changes promptly.
- Attend weekly case conferences and participate in care coordination.
- Identify and address risks such as falls, medication issues, and disease‑management needs.
- Provide education to reduce avoidable hospitalizations.
- Support agency quality metrics and patient satisfaction goals.
- Meet productivity expectations (30 points weekly).
- Respond to urgent staffing and patient‑care needs across the service area.
- Demonstrate flexibility with assignments based on organizational needs.
- Reimbursed between visits only
- Not reimbursed for home → first patient or last patient → home
- Active, unrestricted RN license
- 1–2 years of home health experience
- HCHB experience required
- Valid driver’s license & reliable transportation
- Current BLS certification
