Affinity Hospice is seeking an experienced Registered Nurse for a full-time remote role delivering hospice and palliative care to patients in their homes or care facilities. You will perform comprehensive assessments, develop and implement individualized care plans, manage symptoms, and educate patients and families.
Daily coordination with an interdisciplinary team, accurate documentation, on-call coverage, and participation in quality improvement initiatives are essential.
