Candidates must live in Pennsylvania. Will be a Licensed Practical Nurse performing duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the program. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients. The Nurse Navigator, functioning in the spirit of teamwork, is accountable for the delivery of care utilizing skills learned in both the clinic and didactic arena.

Assists other team members on the unit. The nurse navigator role provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.

This is a work from home position, but candidates must live locally in Pennsylvania. This role is full-time, 40 hours weekly;
Days. A minimum of 2 years LPN work experience is required.

Graduate from Specialty Training Program-Nursing (Required)

Experience:

Minimum of 2 years-Nursing (Required)
