Description
The MDS Coordinator supports the resident assessment process by completing the appropriate sections of the Minimum Data Set (MDS) and related Care Area Assessments (CAAs). This role also helps develop individualized care plans by identifying resident needs and expected outcomes in accordance with federal and state regulations.

Required Qualifications- Must hold a current nursing license in the state of employment.
- Working knowledge of the MDS 3.0 (current version of minimum data set).
Duties and Responsibilities:
- Resident Assessment & Care Planning: Conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for any significant changes in residents' conditions.
- Compliance & Documentation: Ensure accurate completion and submission of MDS assessments in accordance with Medicare/Medicaid guidelines and state/federal regulations.
- Interdisciplinary
Collaboration:
Facilitate care plan meetings, maintain effective communication across departments, and support continuous improvements in resident care outcomes.
