Complex Care Solutions seeks a Nurse Practitioner per diem to deliver in-home and virtual assessments across Middlesex County, MA. You will spend a full hour with members to capture their personal needs, evaluate health status, identify gaps in care, and support care management aimed at keeping patients safe at home and out of the hospital.
Candidates must hold ANCC or AANP certification, an active nursing license, and be able to work autonomously in a remote clinical environment.
