Gentiva Hospice is seeking a Telephonic Triage RN to join our clinical call center and provide after-hours nursing support to hospice patients and families from home. You will assess symptoms, triage calls, guide care decisions, and coordinate with the interdisciplinary team to avoid crises.
Ideal candidates have an accredited nursing degree, an active RN license in a compact state, and hospice or home health experience. Strong communication, documentation, and empathy are essential for success.
