Banner Health is seeking a High Risk RN Case Manager on a Readmission Case Management Team. The role involves managing a caseload of 40-60 high-risk members, with telephonic, home visit, and televisit responsibilities.
Office hours are Monday through Friday, 8:30 AM to 4:30 PM in a remote position with occasional in-person visits in Arizona. Requirements include a BSN, active RN license in Arizona, and BLS certification.
