United Health Group is seeking a Telephone Case Manager RN to support a diverse member population through education, advocacy, and connections to resources. This remote role allows work from anywhere in the United States, with a schedule Monday–Friday and 8 AM–8 PM EST shifts, including some nights.
You will conduct health assessments, identify gaps in care, educate patients for self-management, and coordinate referrals, using strong computer and Windows skills in a high-volume environment.
