LPN Opportunities At Franciscan VNS Home Health And Hospice

Franciscan VNS Home Health And Hospice is looking for licensed practical nurses (LPN) to join their team in Beech Grove, Greenwood, and Greensburg. Full time and PRN positions are available.

LPNs in home healthcare provide direct patient care, educate caregivers, and participate in the patient's care team. This role requires the ability to assess needs, provide interventions, and revise care under the supervision of a registered nurse.

What you can expect:

• Sign on bonus available (ask your Talent Acquisition Advisor if you are eligible)
• Minimum start pay $24.83 per hour
• Monday to Friday, 8am to 5pm
• Weekend and holiday rotation
• Implement nursing plan of care at the direction of the registered nurse
• Provide home-based patient care based on physician orders and the established nursing plan of care
• Collaborate with Home Health RN and multidisciplinary treatment team to individualize each patient's treatment needs
• Conduct patient psychosocial assessments
• Contribute to developing discharge plans
• Document patient care information

Qualifications:

• Associate's Degree or Diploma from Accredited Licensed Practical Nurse Program
  - Required
• 1 Year Acute Care, Long-Term Care or Home Health Experience
  - Required
• 2 years Home Health Experience
  - Preferred
• Licensed Practical Nurse (LPN) - State Licensing Board
  - Required
• Cardiopulmonary Resuscitation (CPR) - American Heart Association
  - Required
• Driver's License
  - Required
• Auto Insurance
  - Required
