Care Talk Health is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for remote care delivery. This full-time W2 hourly position supports CCM, RPM, RTM and related programs under RN supervision, focusing on chronic care management and patient engagement.
You’ll work 40 hours weekly, with core hours EST 9:00am–6:00pm, one Saturday monthly, and rotating holidays. Strong remote communication, documentation in EHR, and HIPAA compliance are essential.
