What You’ll Be Doing:

The Telemedicine Nurse Coordinator and Clinical Informatics Specialist is a dynamic position.  is a growing telemedicine company and the successful candidate for this position will be willing to adapt to new challenges. The candidate will perform a clinical and administrative role responsible for supporting patients receiving outpatient parenteral antimicrobial therapy (OPAT) and managing informatics, reporting, documentation, and billing activities for a busy telemedicine company.

The position has two areas of focus: telemedicine-based OPAT patient monitoring and clinical coordination, and clinical informatics, data reporting, database management, and billing support. The position may be filled by a qualified Registered Nurse (RN) or Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).

  • Support the clinical operations of the organization's OPAT and Tele-Infectious Diseases (Tele-), and other telemedicine programs.
  • Conduct scheduled telephonic or virtual outreach to patients receiving outpatient intravenous antimicrobial therapy or other patient monitoring.
  • Monitor patient-reported symptoms, medication tolerance, vascular access concerns, laboratory completion, treatment adherence, and other indicators identified in the patient's care plan.
  • Review available clinical records, laboratory results, medication information, and physician recommendations within the nurse's authorized scope of practice.
  • Identify abnormal results, changes in condition, adverse medication reactions, missed doses, line complications, or other clinical concerns and elevate them promptly according to established protocols.
  • Assist patients and caregivers with questions regarding treatment plans, laboratory schedules, medication administration, vascular access care, follow-up appointments, and physician recommendations.
  • Coordinate communication among Tele-, primary care providers, infusion pharmacies, home health agencies, laboratories, skilled nursing facilities, and other involved organizations.
  • Obtain and organize medical records, laboratory results, discharge documentation, medication lists, and other information necessary for ongoing OPAT monitoring.
  • Document patient contacts, clinical observations, interventions, escalations, and care coordination activities accurately and promptly in the designated electronic systems.
  • Assist with transitions of care, including hospital-to-home or hospital-to-facility transitions, and help ensure continuity of antimicrobial treatment.
Clinical Informatics, Reporting, and Billing
  • Maintain, validate, and analyze clinical, operational, quality, and billing-related data for OPAT and Telemedicine services.
  • Create and maintain reports, dashboards, worklists, patient registries, and performance-tracking tools using Microsoft Excel and database systems.
  • Use Excel functions such as formulas, filters, lookup functions, conditional formatting, pivot tables, data validation, charts, and data-cleaning tools.
  • Enter, extract, reconcile, and audit information from electronic health records, practice-management systems, billing platforms, and internal databases.
  • Track program metrics such as patient volume, monitoring activity, laboratory completion, adverse events, escalations, hospital readmissions, treatment completion, documentation timeliness, and outcomes.
  • Assist with charge capture, billing reconciliation, claim-support documentation, and identification of missing or inconsistent data.
  • Apply working knowledge of relevant ICD‑10‑CM, CPT, HCPCS, evaluation and management, remote monitoring, telehealth, and other applicable billing codes, as appropriate to the role.
