Cadence Health is seeking a Care Management Registered Nurse to support patients in our APCM program. You will proactively monitor data, evaluate clinical information, and deliver timely interventions to promote patient safety and health outcomes.
This remote role requires a Multi-State RN Compact license and an Associate Degree in Nursing. If you have 5+ years in ICU/ER, experience in chronic care management, and strong EMR skills (Athena/EPIC), you will thrive in our interdisciplinary teams
