Redwood Coast PACE with Humboldt Senior Resource Center is seeking a Nurse Practitioner (part-time or full-time) to provide comprehensive primary care for our elderly participants in California. The position offers regular hours, Monday–Friday, 8 am–5 pm, with in-home visits as needed.
Responsibilities include conducting intake assessments, developing quarterly care plans, coordinating with the Interdisciplinary Team, and coordinating transitions of care.
