Monday - Friday. Eligible to work from home after 6 months of employment.

JOB SUMMARY:
The Nurse Coordinator is responsible for coordinating the care of all Think patients. Serves as the main conduit of the provider's entire clinical team. This role is assigned to a care team and works closely with a multidisciplinary team to provide the best personalized care to the patients ensuring that the appropriate levels of healthcare resources are utilized. This role also develops a patient-specific plan of care for patients enrolled in a specific value-based program.
The Nurse Coordinator will assist each patient in following their plan of care to improve health outcomes and satisfaction and to decrease healthcare costs.
ESSENTIAL JOB FUNCTIONS:
KNOWLEDGE, SKILLS & ABILITIES:
EDUCATION & EXPERIENCE:
Associate Degree in Nursing (ADN) required, Bachelor of Science in Nursing (BSN) preferred. Active Nursing License in the State of Nebraska required. Active BLS/CPR certification required. A minimum of two (2) years of nursing experience required. Bachelor of Science in Nursing (BSN) and two (2) years of experience in case/care management working with adult patients preferred. One (1) year of experience in a triage nursing setting preferred.
WORKING CONDITIONS AND PHYSICAL EFFORT:
• This role operates in a healthcare setting. This position requires frequent sitting and computer work and allows employee to vary physical position or activity for comfort.
• Must be able to:
- Stand 10% or longer of a workday.
- Walk 10% or longer of a workday.
- Sit 80% or longer of a workday.
• Requires ability to lift up to 10 pounds occasionally.
• Requires employee to: bend, squat, kneel, and reach above shoulder level occasionally and twist occasionally.
• Requires repetitive use of hands for: simple grasping, fine manipulation, computer use.
• Requires all sensory skills (speech, vision, smell, touch, and hearing) corrected to near normal range.
