, delivering psychiatric care to unsheltered individuals experiencing serious mental illness and co-occurring substance use disorders. Through field visits and telehealth, you'll meet members where they are - providing evaluations, medication management, and harm-reduction-oriented care, while lending clinical leadership to a dedicated multidisciplinary team. Candidate must be willing to travel throughout Chemung and Steuben Counties on a regular basis.

Grade 313:
This is a part-time position, 8 hours per week, working from home, but does require travel throughout Chemung and Steuben Counties.
The minimum and maximum annual salary that Elm & Oak Health believes in good faith to be accurate for this position at the time of this posting are $75/hr - $90/hr in addition to your hourly wage, Elm & Oak Health offers a 401k plan with employer match, all state and federal mandated benefits, and a kind and supportive working environment (all benefits are subject to eligibility requirements) and non-monetary perks.

POSITION SUMMARY

The Psychiatric Provider is an integrated member of the Safe Options Support team, delivering comprehensive psychiatric care to unsheltered individuals experiencing serious mental illness, substance use disorders, and/or co-occurring conditions. The role encompasses direct clinical care, care coordination with outside providers, clinical leadership, and staff education, consistent with NYS OMH SOS program guidelines. Telehealth and field visits may be used flexibly to improve participant access.

Travel days for case conferencing and staff education are included within the scope of the role.

Caseload Performance Expectations (OMH)
  • Within 6 months of hire — 4 unique individuals
  • End of first year (12 months) — 6 unique individuals
  • Annually thereafter — 14 unique individuals per year
ESSENTIAL JOB DUTIES/FUNCTIONS
  • 40% Direct Psychiatric Care
    • Performs diagnostic evaluations, provides psychotherapy, and implements psychiatric medication regimens for SOS participants when clinically indicated.
    • Utilizes flexible modalities including field visits, engagement encounters, and telepsychiatry to improve access.
    • Implements long-acting injectable medications, Medication Assisted Therapy (MAT), clozapine therapy, and medication delivery support as clinically appropriate.
    • Apply evidence-based, harm-reduction-oriented prescribing practices, including once-daily dosing, short prescription intervals, and long-acting injectables to support adherence.
  • 30% Care Consultation & Coordination
    • Consults, advises, and coordinates with non-SOS psychiatric providers to clarify treatment roles, update diagnoses, and advocate for effective treatment plans.
    • Collaborate with the SOS team to communicate with hospitals and crisis services when indicated.
    • Coordinates care transitions between practitioners.
