Overview

Registered Nurse – Home Health (Hospice) travel assignment in Rutland, Vermont.

Start date:

07/27/2026 for 13 weeks. Weekly pay: $2,488–$2,655 per week. Guaranteed hours: 37 per week. Shift: 5x8 Days. Setting:
Home healthcare – Hospice. This is a travel temporary assignment.

Responsibilities
  • Deliver professional nursing care to home-based patients using the nursing process; assess patient and caregiver needs and develop an evidence-based plan of care.
  • Serve as Case Manager for assigned patients, actively managing caseloads and visit times in line with agency standards.
  • Complete and maintain accurate OASIS documentation in accordance with agency policies and regulatory requirements.
  • Implement the medical plan of care and nursing interventions; maintain infection control standards and timely documentation.
  • Collaborate with LPNs and other disciplines to ensure the plan of care is followed and patient needs are met; participate in IDT meetings as required.
  • Educate patients and caregivers on medications, treatment options, discharge instructions, and safety considerations; coordinate safe discharges or transfers.
Requirements
  • Current, active RN license in Vermont (Vermont State Board of Nursing) or equivalent with eligibility to practice in Vermont.
  • OASIS experience required.
  • Prior home health and hospice experience preferred; strong assessment, care planning, and documentation skills.
  • Proficient in high-tech nursing skills (IV use and maintenance, wound care including wound VAC, complex wound care) and infection control practices.
  • Ability to work effectively in a home-based setting; strong communication and teamwork skills.
  • Some on-call and weekend coverage required.
