Heartland Hospice is seeking an RN Case Manager to lead in home hospice care across Oshkosh/Berlin/Ripon or Manitowoc/Calumet. You will assess patients, develop plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, family-centered care.
Responsibilities include education for families, symptom management, and documentation, with on-call duties as applicable. Current RN license and CPR required, hospice experience preferred.
