Complex Care Solutions (CCS) is seeking a Nurse Practitioner per diem to perform in-home and telehealth assessments for health plans nationwide. The NP will spend a full hour with members to capture needs for home-based care and avoidance of hospital visits.
Responsibilities include comprehensive assessments, evidence-based treatment planning, and collaboration with PCPs and health plans. Licensure, board certification, and HIPAA compliance are required; remote work with some on-site coordination
