RN Nurse
Home-Based Nursing Care Provider
Provides comprehensive home-based nursing care to patients, managing a patient caseload and collaborating with multidisciplinary teams to achieve optimal health outcomes. Performs assessments, educates patients and families, and ensures quality, safe care delivery in accordance with care plans.
Responsibilities:
- Conduct comprehensive initial and ongoing assessments of patients’ needs at scheduled intervals.
- Administer skilled nursing interventions independently and in coordination with the care team.
- Educate patients and families on health management, medication, and safety practices.
- Document patient information accurately and timely, including care plans, responses, and progress.
- Adjust care plans based on ongoing assessments and patient response.
- Collaborate with other disciplines to ensure holistic, patient-centered care.
Qualifications:
- Hold a valid RN license in the state of practice.
- Possess a Bachelor's Degree in Nursing (BSN) preferred.
- Have experience in acute care, home care, or LTC/SNF settings preferred.
- Maintain current CPR/BLS certification from an approved vendor.
- Possess a valid driver's license, auto insurance, and reliable transportation.
- Be at least 21 years of age or older.
