This full-time role is ideal for an RN with experience in Hospice, Home Health, care coordination, and patient-centered care, who thrives in an independent, field-based environment while collaborating with an interdisciplinary team.
What You’ll DoPatient Care & Case Management Perform comprehensive nursing assessments and develop individualized plans of care Coordinate and manage hospice services in collaboration with physicians and care teams
Provide skilled nursing care, treatments, and patient education
Monitor patient progress and communicate changes in condition to physicians
Help ensure high-quality, compassionate hospice services are delivered consistently.
Care Coordination & Documentation Maintain accurate, timely clinical documentation and patient records
Communicate with healthcare providers and community resources
Ensure access to necessary medical equipment and services
Leadership & Supervision Supervise and evaluate Home Health Aides (HHAs), CNAs, and LPNs/LVNsProvide guidance to ensure quality care delivery and compliance
Additional Responsibilities Participate in IDG, quality assurance and performance improvement programs
Attend training and in-service education
Participate in on-call rotation as required What You’ll Need Graduate of an accredited nursing program
Active Registered Nurse (RN) license – Oregon Minimum 2 years nursing experience (hospice, home health or public health preferred)
Current CPR certification
Valid driver’s license and reliable, insured vehicle
Key Skills Strong clinical assessment and decision-making
Excellent communication and documentation skills
Ability to work independently with strong time management
Leadership ability and experience with staff supervision
