Emblem Home Health & Hospice is seeking a dedicated Home Health Admit RN to cover West Valley communities, including Sun City, Surprise, Litchfield Park, Goodyear, and El Mirage. You will perform Start of Care visits, assess patients, reconcile meds, and design individualized care plans for home-based nursing care.
You will also perform wound care, administer IV therapies as ordered, document OASIS accurately, and coordinate with physicians and therapists to ensure seamless patient care.
