RN Afterhours Care Manager

Gentiva is seeking an RN After hours Care Manager to provide overnight support for home-based patients Monday-Thursday. In this role, you'll deliver telephonic triage, assess symptoms, and coordinate urgent interventions to ensure safe, high-quality care after hours. You will collaborate with on-call clinicians, update care plans in the EMR, and communicate with families to support comfort and dignity. The position requires strong clinical judgment, excellent communication, and the ability to work independently while upholding Gentiva's mission of compassionate, patient-centered care.
