About

The Role

We are seeking a dedicated and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in State College, PA. In this hybrid role, you will spend time on-site at skilled nursing facilities, conducting patient assessments and developing personalized care plans. You will also work remotely from home, supported by a dedicated scribe team for efficient documentation. Your success will depend on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.

Responsibilities
  • Conduct patient assessments and develop personalized care plans in collaboration with healthcare team members
  • Provide high-quality preventative care to residents in skilled nursing facilities
  • Document patient information accurately and efficiently using electronic medical records
  • Participate in care planning meetings and collaborate with facility staff to ensure seamless continuity of care
  • Stay up-to-date with changing healthcare regulations and best practices
Requirements
  • Active NP or PA license (in the state of practice)
  • 1+ year of experience in Geriatric care and documentation review
  • Excellent communication, collaboration, and teamwork skills
  • Ability to work independently and as part of a team
