Role Description The Hospice Clinical Supervisor is a full-time, on-site role based in Elmhurst, IL, responsible for overseeing clinical hospice services and ensuring high-quality, person-centered care. This role includes supervising clinical staff, providing clinical guidance, and coordinating interdisciplinary team activities to support individualized care plans. The Hospice Clinical Supervisor reviews and monitors patient assessments, care plans, and clinical documentation to support regulatory compliance and best practices.

Day-to-day responsibilities also involve collaborating with physicians and other healthcare professionals, mentoring team members, and participating in case conferences to address complex clinical and psychosocial needs. The role requires active engagement in quality improvement initiatives, staff training, and communication with patients, families, and caregivers to promote understanding and continuity of care.
Qualifications

  • Strong Supervisory Skills and Clinical Supervision experience to lead and support hospice care teams.
  • Case Management skills to coordinate services, manage caseloads, and ensure continuity of care across disciplines.
  • Current licensure as a Registered Nurse or other relevant clinical license in the state of Illinois.
  • Experience in hospice, palliative care, or related home-based healthcare settings.
  • Knowledge of hospice regulations, documentation standards, and quality improvement practices.
  • Excellent communication, collaboration, and conflict-resolution skills for working with patients, families, and interdisciplinary teams.
  • Ability to prioritize, organize, and manage multiple clinical responsibilities in an on-site environment.
