MJHS Home Care seeks an experienced registered nurse to map out and oversee home-based recovery plans. You will deliver advanced clinical home care services, assess patients, manage medications, and educate patients and families in their home setting.
You will coordinate, implement, and continually evaluate each patient’s care plan, review insurance information, and share pertinent findings with families and professional caregivers. NYS license and CHHA experience are preferred.
