Registered Nurse – Home Health

$2148.80/week –

Job Summary:

Provide skilled nursing care to patients in their homes, focusing on promoting health, managing chronic conditions, and ensuring patient safety and comfort.

Responsibilities:

• Assess patient conditions and develop individualized care plans according to physician orders and patient needs.
• Administer treatments and medications, including PICC lines, ports, trachs, and PleurX systems.
• Educate patients and caregivers on disease management, medication usage, and self-care techniques.
• Complete OASIS documentation accurately and in compliance with Medicare guidelines.
• Maintain documentation and record-keeping in accordance with agency protocols and regulatory requirements.
• Adhere to infection control standards and ensure patient safety during visits.

Qualifications:

• Hold an active MI RN license, with no pending ETA, and meet all licensing requirements.
• Possess at least 1 year of home health nursing experience, including familiarity with Medicare OASIS documentation.
• Demonstrate proficiency with PICC, port, trach, and PleurX patient procedures.
• Be certified in BLS; AHA CPR preferred.
• Have recent Epic EMR experience, with completion of training in orientation.
• Willing to provide proof of high school education.
