The Certified Wound, Ostomy, and Continence Nurse (CWOCN) serves as a clinical expert and consultant in wound, ostomy, and continence care across a network of healthcare facilities. This remote role provides virtual assessment, care planning, staff education, chart review, and quality oversight while collaborating closely with onsite clinical teams.
The CWOCN ensures evidence-based practice, supports regulatory compliance, and promotes optimal patient outcomes through proactive wound management and interdisciplinary communication.
Key Responsibilities Clinical Consultation & Patient Management- Provide remote wound, ostomy, and continence assessments via telehealth platforms, EMR review, photographs, and digital communication.
- Develop individualized evidence
- based care plans for patients with pressure injuries, surgical wounds, moisture-associated skin damage, ostomies, fistulas, and continence issues based on facility needs - Review and interpret clinical data (labs, imaging, risk assessments, documentation) to support providers' decision making.
- Collaborate with physicians, NPs, PAs, RNs, case managers, and therapy teams to optimize patient outcomes.
- Participate in care coordination and provide ongoing followup recommendations.
- Serve as the primary remote WOC resource for multiple critical access facilities.
- Review facility wound logs, weekly wound reports, and risk matrices to identify trends and prioritize interventions.
- Support facility staff with complex wound case troubleshooting
- Collaborate with leadership to address quality-improvement initiatives.
- Reviewing supply needs and recommendations based on current patient data
- Provide competency sessions, and ongoing education on wound prevention, ostomy care, and best practices for nurses and staff members.
- Mentor onsite wound champions or unit-based nurses to expand local expertise.
- Develop digital training materials, guides, checklists, and reference tools.
- Review educational needs with facility educators and team leads regularly to advance wound care competencies.
- Create and support facility protocols and procedures that promote current best practice for wound care.
- Participate in facility wound rounds via video conferencing, reviewing documentation and providing recommendations.
- Monitor progress of high-risk wounds, pressure-injury prevention programs, and skin integrity initiatives.
- Utilize telehealth software, secure messaging, and EMR systems to conduct efficient remote consultations.
- Ensure timely response to consults and wound team needs, and maintain HIPAA‑compliant communication across facilities.
- Participate in improving virtual workflows and digital wound‑management processes.
- Utilizing Microsoft 365 products to communicate with other members of the Roshal Operations and Wound Care Team
- Active RN license in good standing in applicable state(s) (Compact State Licensure Preferred).
- Bachelors of Science in Nursing
- CWCN certification from the Wound, Ostomy, and Continence Nursing Certification Board (WOCNCB)
OR WCC Certification fromthe National Alliance of Wound Care and Ostomy (NAWCO) - Minimum 5 years of clinical wound care experience
