Working remotely in a full-time capacity, the Remote RN Case Manager will coordinate patient care, develop care plans, and collaborate with healthcare providers to ensure optimal patient outcomes.
Key responsibilities- Coordinate and manage patient care by assessing needs and developing tailored care plans
- Collaborate with healthcare providers and patients to ensure effective communication and continuity of care
- Monitor patient progress and adjust care plans as necessary to achieve health goals
- Active RN license in the state of practice
- Minimum of two years of clinical nursing experience
- Experience in case management or care coordination preferred
- Certification in case management (CCM) is a plus
- Proficiency in electronic health records (EHR) systems
