Temporary Assignment | Full-Time Hours Available

Compensation: $57-$60 per hour (based on experience, education, location, and other factors)

** Qualifications*
* To be successful in this role, you should have:

- Active RN License in New York State

- Experience with UAS-NY and/or OASIS assessment tools

- 2+ years of home health, community health, or care management experience

- Experience working with adult and geriatric populations

- Strong computer skills and comfort utilizing telehealth platforms

- Excellent clinical assessment and documentation skills

- Strong verbal and written communication abilities

- Knowledge of long-term care services and support programs

- Ability to work independently and manage a remote workload effectively

** Key Responsibilities*
* In this telehealth-focused role, you will:

- Conduct virtual clinical assessments and reviews

- Review and validate NYS Uniform Assessment System (UAS) assessments

- Support care planning and service recommendations

- Collaborate with interdisciplinary teams to improve patient outcomes

- Identify and address gaps in care through clinical review

- Provide education and guidance to members and caregivers as appropriate

- Maintain accurate and timely documentation

- Utilize clinical judgment to support quality care and program compliance
