Patient Care Manager

Piedmont Healthcare is seeking a Patient Care Manager for Home Health in North Augusta. This role leads and coordinates home-based patient care, overseeing clinical staff and daily operations to ensure safe, high-quality, patient-centered services. The manager develops and monitors care plans, ensures regulatory and documentation compliance, and supports performance improvement initiatives. You will mentor nurses and support staff, collaborate with physicians and community partners, and help shape a culture of respect, innovation, and teamwork while advancing your career through ongoing training and development.

Responsibilities

• Oversee daily operations of home health patient care services in the North Augusta area
• Supervise, mentor, and schedule home health clinical staff
• Develop, implement, and update individualized patient care plans
• Ensure compliance with home health regulations, policies, and documentation standards
• Monitor quality, patient outcomes, and satisfaction, leading improvement initiatives
• Coordinate care with physicians, hospitals, and community resources
• Review and approve clinical documentation, including OASIS and EHR entries
• Support staff training, onboarding, and professional development
• Manage caseloads and resource allocation for efficient service delivery
