Accent Care is seeking a Patient Care Manager / Registered Nurse to lead home health services in Hanford, CA. In this role, you will oversee clinical care delivery, supervise nursing staff, and ensure compliance with strict safety and quality standards. You will perform patient assessments, develop care plans, and coordinate with physicians and pharmacists to support complex medication therapies. Drawing on Hanford Pharmaceuticals' culture of collaboration and innovation, you'll mentor nurses, participate in quality improvement, and help advance home-based care practices.

Responsibilities
- Oversee delivery of home health nursing services to patients, ensuring high-quality, compassionate care in accordance with clinical protocols and regulatory standards.
- Supervise and support registered nurses and care teams, providing coaching, performance feedback, and scheduling to maintain adequate staffing coverage.
- Conduct patient assessments, develop and update individualized care plans, and coordinate multidisciplinary services to meet clinical goals.
- Monitor clinical outcomes, documentation accuracy, and compliance with safety, infection control, and privacy requirements.
- Collaborate with physicians, pharmacists, and other healthcare professionals to optimize treatment plans and medication management.
- Educate patients and families on care plans, medications, and home-based therapies to promote adherence and prevent hospital readmissions.
- Use electronic health records to document care, track progress, and generate required clinical and operational reports.
- Participate in quality improvement initiatives, audits, and training to enhance patient outcomes and operational efficiency.
- Serve as a clinical resource for staff, promoting best practices in home health nursing and patient advocacy.
- Coordinate onboarding and ongoing education for nursing staff, reinforcing company policies, procedures, and quality standards.
