Triwill Group seeks a Telehealth Transitional Care Nurse Practitioner for a full-time, exempt role. You’ll conduct post-discharge visits remotely, perform comprehensive assessments, reconcile medications, and support members and caregivers with HIPAA‑compliant video encounters.
Core hours are 8:00 AM–5:00 PM Central, with rotating weekend coverage as needed. Requires NP licensure in at least three states, national board certification (AANP/ANCC) as FNP/AGNP, and a minimum of two years’ clinical
