Job Summary:

Seeking a part-time Registered Nurse (RN) Nurse Assessor to conduct in-home assessments and evaluate patient health status for long-term care planning. The role involves a combination of in-field and telehealth evaluations to support community-based health services.

Responsibilities:

• Perform initial in-home pre-evaluation visits assessing health status, care needs, and patient preferences.
• Review medical documentation and health referral forms relevant to each case.
• Enter evaluation data accurately into electronic forms and transmit as required.
• Document concerns, conflicting information, or issues identified during assessments.
• Communicate with consumers about care options, supports, and service vendors.
• Coordinate enrollment with managed care health plans through effective communication.

Qualifications:

• Current RN licensure in New York State.
• Minimum of two (2) years of recent home care experience within the last ten (10) years.
• Experience conducting assessments similar to UAS, OASIS, or working in CHHA, LHCSA, or MLTC settings.
• Ability to perform in-home evaluations and work independently in a fast-paced environment.
• Strong organizational skills, interpersonal communication, and technology proficiency.
