RN​/LPN Home Care Coordinator

Element Care is seeking an RN/LPN Home Care Coordinator to support frail seniors enrolled in our PACE program. In this role, you'll coordinate in-home nursing services, conduct assessments, develop and update care plans, and collaborate with an interdisciplinary team to promote safety, independence, and quality of life. You'll educate participants and caregivers, monitor clinical status, document accurately in the EMR, and ensure regulatory and quality standards are met while working within a compassionate, mission-driven culture.

Responsibilities

• Coordinate and oversee home-based nursing services for PACE participants
• Conduct comprehensive in-home assessments and ongoing monitoring
• Develop, implement, and update individualized care plans
• Collaborate with an interdisciplinary team to align medical, social, and supportive services
• Educate participants and caregivers on diagnoses, medications, and self-care
• Document all care activities accurately in the EMREnsure compliance with PACE, state, and federal regulations and quality standards
• Communicate changes in participant status promptly to providers and team members
• Support safe transitions of care between home, hospital, and other settings
• Participate in team meetings and quality improvement initiatives
