Job Summary

13wks, Variable shift, 06/22 start date. RN - Hospice. Talented Medical Solutions. Contract. In-Office | Marysville, CA, United States.

• Provide compassionate hospice nursing care to patients in home or facility settings.
• Assess patient conditions, plan care, and adjust treatments per physician orders.
• Monitor and document patient status, vitals, and symptom management in EHR.
• Collaborate with interdisciplinary team to ensure seamless care delivery.
• Maintain compliance with hospice regulations and quality standards.
• Educate patients and families on disease progression, care plans, and safety.
• Meet productivity targets and contribute to pipeline-based coverage planning.
• Utilize data and metrics to identify care gaps and drive improvements.
• Advise on symptom relief, comfort measures, and end-of-life preferences.
• Participate in continuous professional development and apply learnings to practice.
• Assist in discharge planning and transition support for patients and families.
• Maintain on-site presence with timely, reliable coverage and accessible responsiveness.

• Current RN license with hospice or palliative care certification preferred.
• Experience delivering home-based or facility hospice nursing care.
• Strong clinical assessment, care planning, and medication management skills.
• Excellent communication with patients, families, and interdisciplinary teams.
• Knowledge of hospice regulations, safety standards, and compliance protocols.
• Proficient in EHR documentation and quality metric tracking.
• Ability to meet productivity targets and manage a pipeline-based caseload.
• End-of-life care guidance, symptom relief, and comfort measures expertise.
• Willingness to be on-site, responsive, and adaptable to coverage needs.
• Commitment to continuous professional development and practice improvement.
