Job Description & How to Apply Below

my Place Health is built around a simple but powerful belief: older adults deserve the support they need to live safely, independently, and with dignity in their own communities. As a PACE (Program of All Inclusive Care for the Elderly) organization backed by SCAN Group, my Place Health brings together customized medical care, social activities, and daily support for participants and their families – all under one roof.

Our centers are more than healthcare facilities. They are vibrant community hubs where participants are known by name, valued, and supported as whole people. Behind that experience is a dedicated, interdisciplinary team working together to coordinate care, remove barriers, and improve quality of life for some of the most medically and socially complex populations.

For employees, my Place Health offers the opportunity to do deeply meaningful work in a highly collaborative setting. Team members are encouraged to contribute innovative ideas, and grow alongside a mission that prioritizes compassion, respect, and impact. The result is a culture where people feel connected—to their colleagues, their participants, and the communities they serve.

At my Place Health, work is more than a job. It’s a shared commitment to honoring what matters most.

The Regional Home Care Coordinator is responsible for coordinating home care services and related support for participants to ensure services are delivered safely, effectively, and in alignment with each individualized plan of care. As a member of the Interdisciplinary Team, this role completes home-based assessments, supports care planning, monitors service delivery, coordinates with contracted providers and community-based resources, and provides nursing care within the Licensed Vocational Nurse scope of practice as needed.

The role also supports participant and caregiver education, documentation, quality oversight, and day-to-day coordination of home care services, durable medical equipment, and related needs. This regional position requires regular travel to participant homes and work from the PACE center to support care coordination, documentation, and administrative responsibilities across assigned sites.

Responsibilities
  • Coordinate home care services through contracted agencies to ensure services are delivered in accordance with each participant’s individualized plan of care, authorized service needs, and program standards.
  • Participate as an active member of the Interdisciplinary Team by completing home assessments, reassessments, change-of-condition evaluations, care planning activities, and ongoing review of participant home care needs.
  • Provide nursing care within the Licensed Vocational Nurse scope of practice, including wellness checks, medication administration, wound care, and other assigned clinical services in the home, clinic, and day center as needed.
  • Monitor participant condition and home care service delivery through collaboration with home care staff and external agencies, and report changes in condition, treatment concerns, or adverse reactions to the Registered Nurse, Physician, or other appropriate team members.
  • Educate participants and caregivers on in-home care needs, safety practices, and self-management strategies to promote participant independence, well-being, and continuity of care.
  • Respond to participant and family questions or concerns related to home care services in a timely, professional, and service-oriented manner.
  • Schedule and coordinate home care visits and support related appointment logistics, including collaboration with transportation staff to ensure timely and accurate participant service coordination.
  • Oversee and monitor services provided by contracted home care agencies to support quality, consistency, and compliance with applicable regulatory and organizational requirements.
  • Document all nursing care, care coordination, assessments, and service-related activities in the electronic medical record within established time frames to ensure accuracy, continuity, and regulatory compliance.
