Santa Barbara Cottage Hospital is seeking a remote care coordinator to support members by phone or text. You will conduct outreach, assess needs, develop care plans, and coordinate care with providers while documenting actions in the care management system.
Requires an active LVN/LPN or RN license and 2+ years of relevant experience. Remote work from a dedicated home office is required;
Medicaid experience and strong communication skills are valued. Bilingual Spanish is a plus.
