Complex Care Solutions is seeking a Nurse Practitioner for per diem work to perform in-home and telehealth assessments across assigned zip codes in Adams County, CO. Providers will spend a full hour with members to capture a comprehensive view of needs and plan care accordingly.
Must hold an active NP license with ANCC/AANP certification and CMS/HIPAA familiarity. Remote modality and 25 slots per month (≈37 hours) are expected, with continued documentation and timely communication to PCPs.
