Choice Health At Home is seeking a Home Health RN Case Manager for our Broken Arrow, OK branch. This full-time role focuses on comprehensive home-based nursing care, assessing patient needs, and coordinating multidisciplinary care to promote independence and optimal health outcomes.
The ideal candidate holds an active RN license, CPR, a valid driver’s license, and auto insurance, with 2+ years of Home Health experience preferred. Home Care Home Base experience is a plus.
