Complex Care Solutions (CCS) is seeking a Nurse Practitioner for per diem work delivering in-home and telehealth assessments for health plans nationwide. You will spend time with each member to capture a comprehensive view of needs, risk factors, and care management opportunities to keep members at home and out of the hospital.
An active NP license, ANCC/AANP certification, and CPR are required; home health experience is a plus.
