Weekend Admission Nurse

Part Time Registered Nurse Case Manager

Anticipated Territory: Alameda County:
Hayward/Fremont*

Schedule: Saturday/Sunday

Important Note: All opportunities at 21st Century Home Health Services require visiting patients in their homes. Remote or work‑from‑home positions are not available unless specifically noted.

• Conduct Start of Care (SOC) admissions using OASIS
• Conduct follow‑ups and evaluation visits as needed within the designated territory
• Make home visits to patients in the designated territory
• Perform assigned duties including medication reconciliation, wound care, treatments, and procedures
• Monitor clients’ conditions and report changes to the clinical team
• Collaborate with physicians and agency staff to deliver the best possible care
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided
• Work within a 5–15 mile radius of the assigned territory

• Current and unrestricted California RN license
• Current BLS/CPR certification
• Valid California driver’s license
• Experience conducting assessments and Start of Care (SOC) – OASIS is highly desired
• High level of clinical knowledge in home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
