Schedule:
Saturday/Sunday
- Conduct Start of Care (SOC) Admissions - OASIS
- Conduct follow‑ups and evaluation visits as needed in designated territory
- Make home visits to patients in designated territory
- Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures
- Monitor clients’ conditions and report changes to the Clinical team
- Collaborate with physicians and agency staff to deliver the best care possible
- Accurately document observations, interventions, and evaluations related to patient care management and services provided
- Use concierge resources that assist with time‑consuming administrative tasks
- Create a flexible schedule and determine your own work hours
- Target patient visits within a 5–15 mile radius of the primary territory
- Enjoy autonomy with no clocking in/out
- Field additional coverage occasionally outside the primary territory as needed for staffing or patient census
- Current and unrestricted California Registered Nurse license
- Current BLS/CPR certification
- Valid California driver’s license
- Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
- High level of clinical knowledge in home health care is highly preferred
- Experience with high‑acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
- Self‑directed, empathetic, and a critical thinker
- Outstanding interpersonal skills and leadership ability
- Organized and detail‑oriented
