We are seeking a Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs. This role combines limited field work with remote care coordination, offering strong work-life balance while making a meaningful impact in the community.
What You’ll Do- Conduct in-home assessments to evaluate members’ clinical, functional, and social needs
- Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams
- Coordinate care across home care, hospital, rehabilitation, and long-term care settings
- Complete approximately 6 home visits per week within Brooklyn
- Monitor patient progress and adjust care plans to ensure optimal outcomes
- Support safe discharge planning and reduce hospital readmissions
- Communicate with members, families, caregivers, and providers
- Maintain accurate and timely documentation in compliance with guidelines
- Active, unrestricted RN license in New York
- Associate’s degree in nursing required (BSN preferred)
- Minimum 3 years of RN experience
- Experience in MLTC, home care, geriatrics, or case management preferred
- Strong communication, organizational, and critical thinking skills
- Ability to work independently in a hybrid environment
- Bilingual skills are a plus
Languages:
Spanish or Cantonese/mandarin or Russian preferred
Competitive salary: $100K – $130K per annum
