Aachele Home Care and Hospice, LLC is seeking a compassionate, dependable, and experienced Registered Nurse (RN) to join our hospice team.

We are looking for a nurse who understands that hospice care is about more than completing a visit. Our nurses provide comfort, dignity, education, symptom management, and emotional support to patients and families during one of the most important stages of life.

The ideal candidate is organized, communicates well with the interdisciplinary team, completes documentation on time, and is comfortable providing care in patients' homes and residential settings.

Key Responsibilities
  • Perform skilled hospice nursing visits in the patient's home or residential setting.
  • Complete comprehensive and ongoing nursing assessments.
  • Assess pain, symptoms, functional status, safety concerns, and changes in condition.
  • Provide comfort-focused nursing care and symptom-management interventions.
  • Administer and manage medications and treatments according to physician/practitioner orders and the hospice plan of care.
  • Educate patients and caregivers regarding medications, disease progression, comfort measures, safety, and when to contact hospice.
  • Develop, implement, review, and update nursing components of the individualized plan of care.
  • Communicate changes in patient condition promptly to the appropriate hospice team members and provider.
  • Participate in interdisciplinary group/team meetings and care-plan reviews.
  • Coordinate patient care with physicians, nurse practitioners, social workers, chaplains, hospice aides, facilities, pharmacies, DME providers, and other members of the care team.
  • Provide appropriate hospice aide supervision and ensure aide care is consistent with the patient's plan of care.
  • Complete nursing documentation accurately and within agency-required time frames.
  • Assist with admissions, recertifications, transfers, discharges, deaths, and other hospice clinical processes as assigned.
  • Provide patient and family education regarding hospice services and end-of-life care.
  • Recognize and respond appropriately to changes in condition and symptom-management crises.
  • Follow infection-control, emergency-preparedness, HIPAA, patient-rights, and agency policies.
  • Participate in required orientation, annual education, competency evaluations, and quality-improvement activities.
  • Provide on-call or after-hours coverage when scheduled.
Minimum Qualifications
  • Current, unrestricted Georgia Registered Nurse (RN) license.
  • Graduate of an accredited nursing program.
  • Current CPR/BLS certification as required for assigned duties.
  • Minimum 1 year of professional nursing experience preferred.
  • Hospice, home health, palliative care, geriatrics, or end-of-life nursing experience strongly preferred.
  • Strong clinical assessment and documentation skills.
  • Ability to work independently in a home-based environment.
  • Excellent communication and patient/family education skills.
  • Reliable transportation.
  • Valid driver's license and automobile insurance, as applicable to assigned travel.
  • Ability to successfully complete required background screening and agency onboarding requirements.
  • Ability to use an electronic medical record (EMR) system.

Schedule

PRN / Part-Time

Schedule may include weekday visits, admissions, recertification visits, supervisory visits, and occasional on-call or after-hours assignments based on patient and agency needs.

Compensation

Competitive pay based on experience and assignment.

Additional compensation may apply to admissions, after-hours visits, on-call coverage, or other designated assignments according to agency policy.
