Remote Nurse Practitioner – TCM / CCM

We are seeking an experienced Nurse Practitioner to manage a full patient caseload focused on Transitional Care Management (TCM) and Chronic Care Management (CCM). This is a 100% remote position offering a consistent Monday through Friday schedule.

The ideal candidate is organized, clinically strong, comfortable working independently, and capable of managing a high-volume caseload while ensuring patients receive appropriate follow-up and ongoing care.

Responsibilities:

• Manage a full caseload of TCM and CCM patients
• Conduct remote patient assessments and follow-up visits
• Manage patients with multiple chronic medical conditions
• Review medications, treatment plans, hospital discharge information, and care needs
• Coordinate care following hospital and facility discharges
• Identify changes in patient condition and provide appropriate clinical intervention
• Communicate with patients, families, primary care providers, specialists, pharmacies, and other members of the care team
• Maintain accurate and timely clinical documentation
• Ensure TCM and CCM requirements are completed within required time frames
• Work independently while maintaining strong productivity and quality standards

Requirements:

• Active Nurse Practitioner license
• Active DEA registration
• Active Medicaid and Medicare enrollment/credentials
• Must be comfortable managing a full patient caseload
• Strong clinical judgment and ability to work independently in a remote environment
• Excellent documentation, communication, and time-management skills
• Previous TCM, CCM, primary care, care management, or chronic disease management experience strongly preferred

Compensation &

Schedule:

• $120,000 annual salary
• Full-time
• Monday through Friday
• 100% remote
