Somatus is seeking an RN Care Manager to support complex renal patients in their homes, at dialysis centers, and via telehealth. The role combines remote work with occasional home visits within the region and works with a multidisciplinary team to improve outcomes.
You will conduct assessments, create care plans with nephrologists and PCPs, educate patients on self-management, promote home dialysis and transplantation, and coordinate care across the continuum to reduce readmissions.
