Vivo Care is seeking a Care Navigator to build trusted relationships with patients, triage vital signs, and promote health behavior change. You will work with the care team to conduct wellness calls and develop action plans to improve health and well-being using our remote care platform.
Ideal candidates have an unrestricted RN/LPN/LVN license in a compact state and Massachusetts, experience with RPM or CCM, and comfortable with phone-based visits.
