Gentiva is seeking an After Hours Hospice RN to respond to patient needs during evenings, nights, weekends and holidays. You will provide direct patient care after hours and act as the primary liaison among patients, families, physicians, and the hospice team.
You will work independently, document visits in EMR, and collaborate with an interdisciplinary team to guide plans of care and ensure continuity of home-based hospice services.
