Job Summary:

We're seeking a skilled and dedicated MDS Nurse (hybrid) to join our team at Advinia Care Newburyport. As a vital member of our healthcare family, you'll be responsible for coordinating and completing accurate MDS assessments, working closely with interdisciplinary teams to develop individualized care plans, and ensuring optimal reimbursement through compliant assessments. 1-2 days remote work in this position. • Coordinate and complete accurate MDS assessments per state and federal guidelines.
• Work closely with interdisciplinary teams to develop individualized care plans.
• Manage Medicaid documentation and assist with Medicare admissions.
• Support optimal reimbursement through compliant assessments.
• Keep our resident records organized, timely, and audit ready.

• Valid RN or LPN license (Massachusetts)
• Graduate of an accredited nursing program
• MDS experience required, long-term care experience preferred
• Working knowledge of Medicaid and Medicare documentation
• BLS Certification (approved vendor)
