Position: Registered Nurse (Home Dialysis Coordinator)

Job Title

Duties

Provides direct clinical care during Home Dialysis Clinic: vitals and assessment, IV access and medication administration, quarterly clearance studies (KT/V), and sterile dressing changes

Manages the full new-patient pathway: initial education, home safety/readiness visits, coordination with Vascular Surgery for catheter placement, post-op flush/dressing visits

Conducts required home visits (initial, post-training, and annual) and monthly Home Dialysis Clinic follow-up

Manages supply logistics: monthly inventory and ordering with for each patient and for hospital inpatient use, and monthly Prosthetics consults for cycler rental processing

Oversees monthly labs, lab tracking sheets, lab result letters, appointment letters, and provider clinic packets for each patient

Leads the PD Quality Assurance Performance Improvement (QAPI) program

Runs the IV Iron infusion clinic

Trains new nurses and conducts annual PD competency training for dialysis nursing staff

Preferred

Skills:

Strong independent judgment and organizational skills; demonstrated ability to manage a full caseload of patients across scheduling, clinical care, and administrative tracking with minimal supervision

Patient education and training skills, including the ability to teach complex self-care procedures to patients and families over multi-week training programs

Working knowledge of home safety assessment criteria and infection-control considerations for home-based therapy

Experience with IV therapy, injections, and medication administration in an outpatient/clinic setting

Familiarity with quality improvement processes, data tracking, and root cause analysis methodology

Precepting/teaching ability for training new staff and delivering competency-based education

Preferred Experience:

Prior PD (peritoneal dialysis) experience, hemodialysis experience, more than 3 years of successful nursing practice

Work Schedule:

Monday - Friday, Flexible, Compressed; 10 hour compressed tours available
